اسکلروز منتشر و درمان مکمل در ایران: یک مطالعه مروری

چکیده

مقدمه و اهداف
اسکلروز منتشر، اختلال مزمن سیستم عصبی مرکزی و ایمنی است که پیامدهای جسمی، روانی متعدد و هزینه‏های اقتصادی، اجتماعی زیادی برای بیماران مبتلا و خانواده آن­ها ایجاد می‏کند. ارائه شواهد علمی مبتنی بر فواید درمان‏های مکمل اهمیت ویژه‏ای در مدیریت سلامت و کیفیت زندگی این افراد دارد. مطالعه حاضر به منظور جمع­‏آوری شواهد توصیفی، کاربردی، همچنین مقایسه مداخلات طب مکمل غیرخوراکی در بیماران ایرانی انجام گردیده است.
مواد و روش­ها‏
مداخلات طب مکمل با گروه کنترل که روی بیماران مبتلا به اسکلروز منتشر در ایران صورت گرفته بود، در پایگاه‏های ایرانی شامل؛ مرکز داده‏های پایگاه اطلاعات علمی، پژوهشگاه علوم و فناوری اطلاعات ایران، کتابخانه پزشکی و پایگاه‏های خارجی پاب­مد و گوگل اسکولار، جستجو و جمع‏آوری شد.
یافته‏ ها
پس از بررسی اولیه و حذف مطالعات غیرمرتبط، 41 مطالعه به مرحله تحلیل وارد شد. مطالعات از نظر روش‏شناختی مورد تحلیل و ارزیابی قرار گرفت. مداخلات مناسب بر حسب بیشترین نقاط قوت یا کمترین معیارهای خطا انتخاب شد.
نتیجه‏ گیری
اغلب مطالعات طب مکمل در ایران بر روی بیماران از نوع عود-بهبود صورت گرفته‏اند. خستگی، کیفیت زندگی و خلق، بیشترین متغیرهای ارزیابی شده هستند. انواع ورزش‏ با سطوح و شدت متوسط در مدت کوتاه ولی مداوم مناسب هستند. ورزش‏های هوازی و یوگا به دلیل تأثیر تمرینات هوازی بر تمام اندام‏ها (به ویژه تنفسی) می‏تواند با توجه به تاریخچه بیماری و سطح خستگی توصیه شوند.

چکیده [English]

Background and Aim: Multiple sclerosis is an immune disorder which involves central nervous system. This chronic condition has physical and psychological consequences causing economical and social burdens for patients and their families. Providing scientific evidences based on treatment benefits would have significant importance in health management and patient's quality of life.
The present study aimed to collect descriptive and applied evidences comparing the efficacy of different non-biologic supplementary medicine intervention in Iranian multiple sclerosis individuals.
Materials and Methods: In the present study, we investigated the complementary medicine interventions for multiple sclerosis. Articles were searched in Iranian databases including IranDoc, Medlib, and Magiran as well as international databases including Pubmed and Google Scholar.
Results: After initial screening and deleting irrelevant studies, 41 studies were chosen for the analysis. Studies were assessed and analyzed methodologically. Proper interventions were selected according to the least error criteria or the degree of strength.
Conclusion: Most studies were performed on relapsing-remitting kind of multiple sclerosis. They focused on fatigue level, quality of life, and mood promotion evaluation. Exercises with moderate level and intensity can be used monitoring short intervals and with short duration. Aerobic exercises have positive effects on all body organs as well as respiratory capacity and function; thus, they can be applied based on patient history and fatigue level.
