تأثیر برنامه بازگشت به اجتماع (CRP) بر وضعیت روانی بیماران روانی مزمن مراکز توانبخشی روزانه شهر همدان

چکیده

مقدمه و اهداف
برنامه بازگشت به اجتماع یکی از برنامه­های جدید توانبخشی است که با تجهیز شرکت‌­کنندگان به مهارت­ها و اطلاعات لازم، آنها را قادر می­سازد از شرایط بستری در بیمارستان به زندگی مستقل در جامعه وارد شوند. هدف از مطالعه حاضر، تعیین تاثیر برنامه بازگشت به اجتماع بر وضعیت روانی بیماران روانی مزمن بود.
مواد و روش­ها
40 بیمار مبتلا به اختلالات روانی مزمن مراجعه کننده به مراکز توانبخشی روزانه شهر همدان، به صورت تصادفی در دو گروه آزمون (20 مورد) و کنترل (20 مورد) قرار گرفتند. گروه آزمون برنامه بازگشت اجتماع را در 16 جلسه آموزشی 60-90 دقیقه­ای، به صورت 2 روز در هفته به مدت دو ماه دریافت کردند و گروه کنترل هم در طی این مدت از خدمات کاردرمانی معمول مرکز استفاده کردند. بیماران در هر دو گروه آزمون و کنترل قبل و بعد از اجرای برنامه آموزشی توسط مقیاس مجموعه علائم مثبت و منفی (PANSS) مورد ارزیابی قرار گرفتند.
یافته­ها
یافته­ها بیانگر این است که بیماران گروه مداخله در مقایسه با گروه کنترل پس از اجرای برنامه بازگشت به اجتماع در ابعاد علائم منفی (04/0P=)، آسیب­شناسی روانی عمومی (01/0P=) و نمره کل (02/0P=) از مقیاس مجموعه علائم مثبت و منفی تغییر معناداری را نشان دادند و در بعد علائم مثبت ارتباط بین دو گروه معنادار نبود.
بحث و نتیجه­گیری
بر اساس یافته­های پژوهش حاضر، برنامه بازگشت به اجتماع باعث مهار و بهبود علائم منفی، آسیب­شناسی روانی عمومی و نمره کل وضعیت روانی بیماران روانی مزمن شد، این حاکی از تاثیر برنامه در بهبود وضعیت روانی این بیماران بود. البته برای بررسی نتایج درازمدت برنامه در این بیماران نیاز به مطالعات پیگیری می­باشد.

چکیده [English]

Background and Aim: Community Re-Entry Program (CRP) is one of the new programs that equips participants with the skills and information necessary to enable them to meet the requirements of hospital stay, to live independently in their community. The present study aimed to assess the effect of Community Re-Entry Program (CRP) on the mental state of patients with chronic mental disorders.
Materials and Methods: In the present study, 40 patients with chronic mental disorders referring to outpatient rehabilitation centers of Hamadan, Iran, were randomly divided into two intervention (N=20) and control (N=20) groups. The intervention group received CRP in 16 educational sessions, each lasting for 60-90-minutes, held twice a week for two months. The control group, on the other hand, took part in the routine occupational therapy of the rehabilitation center. Both groups were evaluated using the Positive and Negative Symptoms Scales (PANSS) questionnaire before and after the intervention.
Results: The results showed a significant change in negative symptoms (P=0.04), general psychopathology (P=0.01), and total score of PANSS questionnaire (P=0.02) in the intervention group compared with those of the control group, after the intervention. However, no significant difference was found between the two groups concerning positive symptoms.
Conclusion: According the research findings, CRP could reduce negative symptoms, general psychopathology, and total scores of mental state in chronic mental patients, which reflects the impact of the program in improving mental state in these patients. Yet, future studies are required to assess the long-term clinical effects of this program.
