تأثیر تمرین ترکیبی (تعادلی، قدرتی، انعطافی و راه رفتن) بر تعادل و احتمال افتادن زنان سالمند مبتلا به نوروپاتی محیطی دیابتی

چکیده

مقدمه و اهداف
عوارض مستقیم و غیرمستقیم نوروپاتی محیطی دیابتی می‎تواند به اختلال تعادل و افتادن منجر شود و احتمال بروز زخم پا را افزایش ‎دهد. از طرفی دیگر، انجام تمرینات موثر در افزایش تعادل، احتمالاً بتواند باعث کاهش خطر افتادن در این بیماران ‏شود؛ لذا هدف مطالعه حاضر، بررسی تأثیر تمرین ترکیبی بر تعادل و احتمال افتادن در زنان سالمند مبتلا به نوروپاتی دیابتی بود.
مواد و روش­ ها
در مطالعه حاضر، 23 بیمار زن مبتلا به نوروپاتی محیطی دیابتی به صورت تصادفی به دو گروه مداخله (11 نفر) و کنترل (10 نفر) تقسیم شدند. قبل و بعد از برنامه تمرینی، تعادل و احتمال افتادن آزمودنی‎ها به ترتیب با ابزار تعادل برگ و مقیاس بین‎المللی افتادن اندازه‎گیری شد. گروه تمرین به مدت 8 هفته، هفته‎ای 3 جلسه و 1 ساعت در هر جلسه به انجام تمرین ترکیبی پرداختند. گروه کنترل در این مدت به زندگی معمولی خود ادامه دادند. داده‏ها با استفاده از آزمون‏‎های تی مستقل و وابسته در سطح معنا‏‏داری 05/0>α تجزیه و تحلیل شد.
یافته ­ها
نتایج تحقیق حاضر نشان داد که در نمرات تعادل و احتمال افتادن گروه تجربی بعد از اجرای تمرین ترکیبی نسبت به قبل از آن، تفاوت معناداری ایجاد شد، در حالی‏ که این نمرات در گروه کنترل تفاوت معناداری نداشت.
نتیجه ­گیری
انجام تمرین ترکیبی باعث بهبود تعادل و کاهش احتمال افتادن زنان سالمند مبتلا به نوروپاتی محیطی دیابتی می‏شود؛ بنابراین به منظور بهبود تعادل و کاهش احتمال افتادن این بیماران مداخله درمانی مفیدی است.

Background and Aim: Direct and indirect complications of diabetic peripheral neuropathy can lead to imbalance and falls, and it may increase the risk for plantar ulcer. Therefore, performing effective trainings to increase balance probably reduces the risk of falling in these patients. Thus, the aim of the present study was to investigate the effect of combined training on the balance and chance of falling in elderly women with diabetic neuropathy.
 
Materials and Methods: A total of 23 patients with diabetic peripheral neuropathy were randomly allocated to experimental (n=11) and control (n=10) groups in the present study. Before and after training, balance and risk for falling were measured using Berg balance score and fear of falling tests, respectively. Experimental group was involved with combined exercise for 8 weeks, 3 sessions per week, and 1 hour per session; the control group did not participate in any exercise program. Data were analyzed running independent and paired- sample t tests at the significance level of α  
Results: The results showed that there were significant differences in balance and risk for falling scores in the experimental group after combined training, while there were no significant differences in these scores in the control group.
Conclusion: Combined trainings improve balance and reduce the possibility of falling elderly women with diabetic peripheral neuropathy. Therefore, it is a beneficial therapeutic intervention in order to improve the balance and reduce the risk of falling in these patients.
