بررسی اثرات روش های اکتیو ریلیز و پوزیشنال ریلیز براختلالات عضلانی–اسکلتی: مقاله مروری

چکیده

مقدمه و اهداف
قرار گرفتن طولانی مدت در وضعیت­های نامطلوب، ضرباتی که به­دنبال تصادفات وآسیب­های ورزشی رخ می­دهد، منجر به آسیب بافت نرم می­شود. درمان­های متعددی شامل دارودرمانی، جراحی و فیزیوتراپی مانند درمان دستی، تمرین درمانی به­طور معمول جهت درمان این آسیب­ها استفاده می گردد. آزادسازی بافت نرم شامل انواع فعال و وضعیتی می باشد که جهت کاهش اسپاسم­های عضلانی موضعی استفاده می­شود. تا به امروز به مطالعه مروری در این زمینه پرداخته نشده است. هدف از مطالعه حاضر، بررسی اثرات هر یک از تکنیک­های پوزیشنال و اکتیو ریلیز بر یکدیگر و بر آسیب­های بافت نرم می­باشد.
مواد و روش ها
طی جستجوی مقالات چاپ شده ازسال 2000 تا 2015 در زمینه­ی روش های اکتیو و پوزیشنال ریلیز در موتور جستجوهای Pubmed، Googlescholar، siencedirect، scopus، بامقایسه ART و PRT درمقابل هم یا با سایر روش­های دیگر مورد بررسی قرار گرفتند. تنها مطالعاتی که بر روی بیماران که به زبان فارسی و انگلیسی بود، وارد شد. مقالات کنفرانسی حذف شدند. بیست و هفت مقاله شامل 8 گزارش موردی، 8 کارآزمایی بالینی، 5 مطالعه آزمایشی، 3 مطالعه با طراحی قبل و بعد، و 2 مطالعه­ی مقایسه­ای نیز یافت شد.
نتیجه­ گیری
نتایج مطالعه مروری حاضر نشان می­دهد که تکنیک­های دستی پوزیشنال ریلیز و اکتیو ریلیز، به­صورت کوتاه مدت می تواند سبب کاهش درد، بهبود دامنه حرکتی و فعالیت عملکردی شود، و همچنین در کنار سایر روش های فیزیوتراپی به­عنوان یک درمان ایمن غیرتهاجمی مورد استفاده قرار بگیرد.

Background and Aim: Prolonged bad postures or traumas following accidents or sports injuries may result in soft tissue damage. A variety of treatment protocols including medical, surgical, and physical treatments comprising electrical, manual, and exercise therapies are routinely used for this purpose. Soft tissue releases including active and positional types are recently used to relieve local muscle spasms. To date, no study has been reported reviewing the findings of the researchers in this area. The current library-based study aimed to deliberately focus on this area to find out if any of these techniques is preferred to other one in soft tissue damage.
Materials and Methods: PubMed, google scholar, scopus, and sciencedirect databases were searched from 2000 to 2015 for the studies investigating the comparative effect of ART and PRT versus control group and other routine treatments. Only the papers that studied Active or Positional release techniques on patients and were written in Persian or English languages were included. Conference proceedings were excluded from the study.
Results: A total of 27 relevant papers were found: one review article, eight case reports, eight clinical trials, five pilot studies, three studies with before-after techniques, and two comparative studies.
Conclusion: The results showed that PRT and ART manual techniques can reduce pain and improve ROM, and functional activities in short time. It was also found that these techniques can be used as a non-invasive and safe treatment in treating patients in addition other physical modalities.
